Authorities are investigating a deadly UPS MD-11F cargo plane crash near Louisville Muhammad Ali International Airport on November 4, 2025. The left engine reportedly detached during takeoff, causing the plane to lose control and crash just beyond the runway, igniting massive fires. Twelve people died, including three crew members and nine on the ground.
CCTV confirmed the engine separation, and investigators have recovered the black boxes. Officials are examining potential mechanical failures, including metal fatigue or improper mounting. Flights at the airport were suspended, and local authorities advised shelter-in-place as crews managed hazardous debris.
